CENTRAL ACCREDITATION REQUEST PAGE

Do you want to credit your skull base surgery center and become part of the SIB referral centers? To apply for accreditation to the SIB Board, fill in the following form and attach the CV (European format) of the person submitting the request and a brief presentation of your skull base center. After submitting the request, you will receive a message confirming receipt to the indicated email address. The SIB Board will analyze the accreditation requests and keep you informed about the outcome of the accreditation process. If your center will be accredited, it will be included in the list of the referral centers.
